Insurance Policy Information Requests
Automobile Liability Limits Requests:
Pursuant to § 10-3-1117, C.R.S., insurers shall provide, within 30 days of receipt of a request, the following information:
- The name of the insurer.
- The name of each insured party, as the name appears on the declarations page of the policy (the declaration page itself is not required).
- The limits of the liability coverage of the policy.
- A copy of the automobile insurance policy, which does not include the declaration page or application if attached to the policy.
Homeowner Certified Policy Requests:
Pursuant to § 10-3-110.8, C.R.S., insurers shall provide a certified copy of an insured’s homeowner insurance policy within 30 days of receipt of the policyholder’s request.

If the Division of Insurance is identified as the insurer’s registered agent, click on the link above to complete the liability limits request form. If the Division has an e-mail address on file, the request will be sent immediately to the insurer. If there is no email address on record, the Division will forward the request to the insurer as soon as possible.
If the Division of Insurance is not the insurer’s registered agent, you may download the pdf form, complete it and mail it to the registered agent listed.
