The Division would like to provide notice that the Commissioner has adopted the following amended regulation:
The purpose of this regulation is to promulgate rules concerning the uniform individual and small group health benefit plan applications.
The applications included as appendices to the regulation have been amended to include fields allowing for the collection of a dependent child's address if the address is different from the individual applying for coverage on the individual application, and a section regarding the certification of dental insurance coverage on the small group application.
This regulation will become effective on November 1, 2021.
